Past and Current Grantees

2020

COVID-19 Response Initiative

In March of 2020, Destination: Home launched a series of efforts focused on addressing the impacts of COVID-19 on vulnerable households and communities in Santa Clara County. The work has ranged from direct financial assistance for families and individuals impacted by the crisis to emergency grants for non-profit organizations serving on the front lines. To date, the initiative has disbursed the following grants:

Sacred Heart Community Service

$10,900,000 for a coordinated intake and assessment process that provided direct financial assistance to 4,000 low income households experiencing financial hardships as a result of the COVID-19 pandemic

Family Independence Initiative

$1,000,000 to launch a pilot technology platform to provide direct financial assistance payments to 1,500 extremely low income individuals who are ineligible for CARES Act stimulus support or unemployment benefits

Grassroots Grantee Program

$1,700,000 to provide direct financial assistance resources and related administrative expenses for 50 grassroots and community based organizations serving hard-to-reach and underserved populations throughout the region.

Emergency Assistance Grants

$250,000 in flexible funding for non-profits providing direct services and support to unsheltered and vulnerable people

Supportive Housing & Extremely-low Income Affordable Housing Development

Charities Housing

  • $1.2M for pre-development work at a site that will host up to 73 units of extremely low income and supportive housing in San Jose
  • $2.4M for pre-development work at a site that will host up to 124 units of extremely low income and supportive housing in Santa Clara

MidPen

$2.5M for pre-development work to support a unique partnership with Immanuel Evangelical Lutheran Church to develop up to 106 units of permanent supportive housing on the church’s property.

County of Santa Clara, Office of Supportive Housing

$4M to support a master lease for a 91-unit interim housing development in San Jose.

Danco

$3M for predevelopment work at a site that will host up to 156 units of extremely low income and supportive housing in San Jose

Jamboree

$175,000 in acquisition funding for a site that will host up to 134 units of extremely low income and supportive housing in Milpitas

First Community Housing

  • $5M for acquisition and predevelopment work at a site that will host up to 66 units of extremely low income and supportive housing in Morgan Hill
  • $2.5M or pre-development work at a site that will host up to 101 units of extremely low income and supportive housing

CORE

  • $962,010 to provide funding for rehabilitation of 50 apartments for supportive housing at Markham Plaza 1

BitFocus

$943,000 to develop and launch a client-facing HMIS portal for homeless and formerly homeless individuals.

Supportive Housing & Extremely-low Income Affordable Housing Development

Charities Housing

  • $3.9M to provide funding for the acquisition and pre-development of a site for up to 90 units of extremely low income and permanent supportive housing in San Jose
  • $1M to support pre-development work at a site for up to 80 units of extremely low income and supportive family housing in Mountain View
  • $450,000 for a three-year capacity building grant to fund project management staff and increase the organization's development pipeline for PSH/ELI development in Santa Clara County

Path Ventures

  • $1.8M to support pre-development work at a site for 95 supportive housing units for seniors in San Jose
  • $450,000 for a three-year capacity building grant to fund project management staff and increase the organization's development pipeline for PSH/ELI development in Santa Clara County

Reed Community Partners

  • $1.8M to support pre-development work at a site for 84 extremely low income and supportive housing units in San Jose

Abode Services

  • $1.5M to support pre-development work at a site for 70 extremely low income and supportive housing units for seniors in San Jose
  • $450,000 for a three-year capacity building grant to funding project management staff and increase the organization's development pipeline for PSH/ELI development in Santa Clara County

First Community Housing

  • $450,000 for a three-year capacity building grant to fund project management staff and increase the organization's development pipeline for PSH/ELI development in Santa Clara County

CORE

Homelessness Prevention & Housing Stability

Jobtrain, Inc.

  • $486,074 to serve as the lead organization working with partner agencies to develop and implement a supportive housing employment service system in Santa Clara County.

Data & Technology Utilization

Varsity Technologies

  • $137,000 to implement hardware and software solutions at Sacred Heart Community Service and one additional “Lighthouse” partner agency

Abode Services

$250,000 to provide site acquisition support for 80 new units of supportive housing at Corvin Apartments in Santa Clara

City of San Jose

$540,000 to provide a three-year grant for a dedicated planner to expedite and streamline applications for new supportive housing developments in San Jose

Danco Communities

$1,000,000 to provide site acquisition support for up to 80 new apartments of supportive and ELI housing in San Jose

First Community Housing

$150,000 to provide furnishings for 134 new units of supportive housing at Second Street Studios in San Jose

Data & Technology Utilization

HomeBase

$105,000 to conduct a technology needs assessment of Santa Clara County’s supportive housing system to identify opportunities to leverage data and technology to strengthen the system of care

Homelessness Prevention & Housing Stability

Sacred Heart Community Service

$1,500,000 to expand annual homelessness prevention system capacity to serve 480 households in the second year of program implementation and add targeted services for victims of domestic violence
