Community Plan to End Homelessness
Our community’s roadmap for preventing and ending homelessness — built by the community, for the community.
Since 2005, Santa Clara County has come together across jurisdictions, agencies, and sectors to build a coordinated, evidence-based response to our region’s homelessness crisis. With the Community Plan to End Homelessness, we turn that collective commitment into action.
In the last decade alone, the strategies outlined in our Community Plans have resulted in more than 35,000 people permanently housed, the creation of a Homelessness Prevention System that’s grown to reach more than 44,000 people, and a shelter system that’s doubled in capacity
The 2020-2025 Community Plan to End Homelessness has served as the latest roadmap for addressing our homelessness crisis. Built upon input from thousands of community stakeholders, and grounded in evidence-based practices and lessons learned, the plan was organized around three main strategies:
Strategy 1
Address the root causes of homelessness through system and policy change
Strategy 2
Expand homelessness prevention and housing programs to meet the need
Strategy 3
Improve quality of life for unsheltered individuals and create healthy neighborhoods for all

SYSTEM PERFORMANCE
This progress is helping us narrow the gap between how many households are becoming homeless, and the number of households exiting homelessness. We are curbing the growth of the crisis in our community, but we still have work to do to turn the tide on ending homelessness.
Every member of our community deserves a safe and stable home—and it is our collective responsibility to make this vision a reality.
